In the majority of cases where this issue has been reported, the problem was caused by fast-forwarding through the module before answering the knowledge questions.  

If you fast-forward through a module the learning management system cannot detect that the module has been completed. 

Please relaunch the module and complete it without fast-forwarding to see if that resolves the problem.
